As much as I love the kind of organisation you're describing in this post, I see a lot of leaders struggling with the idea of providing freedom, letting information flow and fostering productive failiing. Creating this kind of environment requires putting your trust in the people instead of using processes and structures to control them. Trusting the people you work with quite often seems to be the missing prerequisite.
